One relatively new service in the United States that can help keep older people in their homes longer is respite care.[18] This type of care allows caregivers the opportunity to go on vacation or a business trip and know that their family member has good quality temporary care, for without this help the elder might have to move permanently to an outside facility. Another unique type of care cropping in U.S. hospitals is called acute care of elder units, or ACE units, which provide "a homelike setting" within a medical center specifically for older adults.[19]
A local example of the life-saving stroke care making a difference was a man who came to Marshall North ER in June with weakness and impaired speech. Tests showed the patient to be a candidate for teleneurology and possible tPA – a clot busting medication. Through the teleneurology system – which links rural hospitals with a neurologist at Huntsville Hospital using a computer monitor – the medical team determined tPA should be administered. The patient received the drug and, after close monitoring, was transferred to Huntsville Hospital.
Not every warning sign will occur in every stroke. And even if they do go away, these warning signs should not be ignored. A TIA (transient ischemic attack) is sometimes referred to as a mini-stroke, and produces symptoms similar to a stroke that only last for a short time. But TIA symptoms serve as an important warning that a stroke could be imminent, and it’s important to respond the same way to a TIA as you would to stroke symptoms.
tPA improves the chances of recovering from a stroke. Studies show that patients with ischemic strokes who receive tPA are more likely to recover fully or have less disability than patients who do not receive the drug.2,3 Patients treated with tPA are also less likely to need long-term care in a nursing home.4 Unfortunately, many stroke victims don’t get to the hospital in time for tPA treatment. This is why it’s so important to recognize the signs and symptoms of stroke right away and call 9-1-1.

Older adults that require assistance with activities of daily living are at a greater risk of losing their independence with self-care tasks as dependent personal behaviours are often met with reinforcement from caregivers.[42] It is important for caregivers to ensure that measures are put into place to preserve and promote function rather than contribute to a decline in status in an older adult that has physical limitations. Caregivers need to be conscious of actions and behaviors that cause older adults to become dependent on them and need to allow older patients to maintain as much independence as possible. Providing information to the older patient on why it is important to perform self-care may allow them to see the benefit in performing self-care independently. If the older adult is able to complete self-care activities on their own, or even if they need supervision, encourage them in their efforts as maintaining independence can provide them with a sense of accomplishment and the ability to maintain independence longer.[43]

A less restrictive alternative to legal incapacity is the use of "advance directives," powers of attorney, trusts, living wills and healthcare directives. The person who has such documents in place should have prepared them with their attorney when that person had capacity. Then, if the time comes that the person lacks capacity to carry out the tasks laid out in the documents, the person they named (their agent) can step in to make decisions on their behalf. The agent has a duty to act as that person would have done so and to act in their best interest.

We at NYU Langone’s Comprehensive Stroke Care Center are fortunate to have recently opened the Ronald O. Perelman Center for Emergency Service, which serves as the first line of urgent care for patients that are suspected of having a stroke. A stroke alert prompts a team of experts from our center to come to a patient’s bedside within minutes to confirm or rule out a stroke, allowing treatment and rehabilitation to begin as soon as possible.
On both occasions I used their service, the caregivers either left minimal or no notes while visiting my elderly parents (no eating, sleeping or bathroom data). After the first visit, I contacted a supervisor at Around the Clock and asked specifically that the caregivers leave notes on eating, sleeping patterns, etc. Instead of improving, on the 2nd visit I only received notes from 1 of 3 caregivers...I paid for 16 hours of caregiving for people who may or may not have even showed up for all I know. Their medical staff is also hit and miss. On my father's post-hospital rehab, one of the 3 nurses missed appts at the house repeatedly...only making 1 of 5 scheduled appointments.
