After a stroke, you may need rehabilitation (rehab) to help you recover. Before you are discharged from the hospital, social workers can help you find care services and caregiver support to continue your long-term recovery. It is important to work with your health care team to find out the reasons for your stroke and take steps to prevent another stroke.
Disposition: Disposition is the last link in the chain of survival. This step in stroke care focuses on the continuing care of the stroke patient. It is recommended that patients be admitted to an intensive care unit or stroke unit within three hours of arrival in the ED. Continued monitoring of a stroke patient includes frequent assessment of neurological status and monitoring of glucose levels and vitals, as well as prevention of complications. Determining the cause of the stroke is also part of disposition.
Data: Data collection is a vital component of the chain of survival. Data collection includes results from laboratory tests and both a physical and a neurological exam. A 12-lead electrocardiogram is recommended to rule out cardiac arrhythmias. In addition, a CT scan is an essential piece of the puzzle and is needed for an accurate diagnosis. A CT scan should ideally be performed within 25 minutes of arrival in the emergency room. Data collected is also needed to rule out conditions which may mimic a stroke, such as a brain tumor, drug overdose or hypoglycemia.
One relatively new service in the United States that can help keep older people in their homes longer is respite care.[18] This type of care allows caregivers the opportunity to go on vacation or a business trip and know that their family member has good quality temporary care, for without this help the elder might have to move permanently to an outside facility. Another unique type of care cropping in U.S. hospitals is called acute care of elder units, or ACE units, which provide "a homelike setting" within a medical center specifically for older adults.[19]

(which was his same attitude in regards to the other facilities) almost a year later, he now considers Best Personal Care his home away from home! The staff is professional and genuine whose main focus is the client's well being. When someone is having symptoms of a stroke – slurred speech, sudden leg or arm weakness, facial drooping, loss of balance or visual changes – getting them to the hospital quickly can mean the difference between recovery and permanent disability. One of the best treatments for ischemic stroke is treatment with the clot-busting drug, intravenous tissue plasminogen activator, or IV tPA. If given in the first three hours after the start of stroke symptoms, IV tPA has been shown to significantly reduce the effects of stroke and lessen the chance of permanent disability. UofL Hospital Stroke Center staff strive to deliver IV tPA to appropriate patients within 45 minutes to one hour from the time they arrive at the hospital.

Round the clock care provided a young female home health aide "APPLE". on her first day she appeared very caring and offered to adjust all my sister-in-laws medication. She stated that she was trained in medication, and gave us her phone number to keep us posted. On the third day she was texting myself and my wife all day while we were working. When i returned home i checked the medication as i always do, and noticed that 60 controlled pain medication was missing. When i called her to let her know she was terrified stating that her finger prints were all over every bottle but she didn't take them. When i called to complain they Round the Clock staff were rude and dismissing. I asked to speak to a supervisor they never returned my call. When i called back they just asked if i wanted someone else to come into my home, when i stated no, they became aggressively rude. I will never allow any home health agency into my home. They have ruined the experience for me. Unprofessional, and criminal rating for Round the clock.
