The form of care provided for older adults varies greatly among countries and is changing rapidly.[2] Even within the same country, regional differences exist with respect to the care for older adults.[3] However, it has been observed globally, older people consume the most health expenditures out of any other age group,[4] observation that shows comprehensive eldercare may be very similar. One must also account for an increasingly large proportion older people worldwide, especially in developing nations, as continued pressure is put on limiting fertility and decreasing family size.[5]

Thank you for assisting our son get his life back under control. When he came out of the hospital I didn't know what to do with him. Your personal interest and that of all the staff and residents at Best Personal Care made him welcome and gave him structure and community he didn't have living on his own. Unlike other places our son has been, I have heard practically no complaints from him about the staff, the food or the other residents.
In most western countries, care facilities for older adults are residential family care homes, freestanding assisted living facilities, nursing homes, and continuing care retirement communities (CCRCs).[8] A family care home is a residential home with support and supervisory personnel by an agency, organization, or individual that provides room and board, personal care and habilitation services in a family environment for at least two and no more than six persons.[9]

A stroke, also called a brain attack, occurs when blood flow to the brain is disrupted. This disruption is caused when either a blood clot block one of the vital blood vessels in the brain (ischemic stroke), or when a blood vessel in the brain bursts, spilling blood into surrounding tissues (hemorrhagic stroke). The brain needs a constant supply of oxygen and nutrients in order to function and its cells begin to die after just a few minutes without either.
It’s very common to have a negative attitude after a serious illness, and some patients experience depression after a stroke — sometimes weeks or months later. However, it is absolutely critical to keep a realistic or positive attitude during rehabilitation to maximize results. Psychologists, psychiatrists and other therapists, who are part of your care team, may recommend therapy and/or medication.

According to the United States Department of Health and Human Services the older population—persons 65 years or older—numbered 39.6 million in 2009.[11] They represented 12.9% of the U.S. population, about one in every eight Americans.[11] By 2030, there will be about 72.1 million older persons, more than twice their number in 2000.[11] People 65-plus years old represented 12.4% of the population in the year 2000, but that is expected to grow to be 19% of the population by 2030.[11] This will mean more demand for elderly care facilities in the coming years. There were more than 36,000 assisted living facilities in the United States in 2009, according to the Assisted Living Federation of America[12] in 2009. More than 1 million senior citizens are served by these assisted living facilities.[12]
Drug/Device: Drug administration, if appropriate, is the next link in the chain of survival. If the patient is a candidate for fibrinolytic therapy, the window of opportunity for administration is narrow. According to the American Heart Association guidelines, fibrinolytic therapy should be administered within three hours of the onset of symptoms. If the patient is not a candidate for drug therapy they may qualify for Endovascular therapy to remove the clot mechanically rather than with fibrinolytics.
Removing the clot with a stent retriever. Doctors may use a catheter to maneuver a device into the blocked blood vessel in your brain and trap and remove the clot. This procedure is particularly beneficial for people with large clots that can't be completely dissolved with tPA, though this procedure is often performed in combination with intravenous tPA.
Marshall Medical’s Stroke Network team includes the physicians and staff of the ED who are the first line of treatment once a patient enters the ED with stroke symptoms. Alongside them are Stroke Coordinator Ken Atchison, and Quality Data Analyst-RN Allison Trammell who work to promote the evidence-based guidelines and assist the ED to ensure they are familiar with any new updates. This team’s effort in promoting evidence-based guidelines and providing the very best treatment possible to patients coming in to the ED is worthy of recognition.
Dispatch: The second step in the chain of survival is ‘dispatch’, which involves activating emergency medical services. In most cases, this involves calling 911. Medical dispatch should be thoroughly trained to identify a possible stroke patient so that the appropriate level of EMS services can be dispatched to the patient. Simply stated, the faster EMS can be dispatched, the quicker lifesaving treatment can be delivered.
